BLIV — CUSTOMER CREATOR PROGRAM // Pronounced "Bleev" WHO THIS IS FOR Current and former Bliv patients who are willing to talk honestly about what exploring care was actually like. WHAT WE'RE ASKING FOR One short-form vertical video, 20–60 seconds. First person, your voice, your story. Not a testimonial and not a review — a story about a decision you made and what the experience was like. WHO'S WATCHING Adults researching convenient online care, including people exploring GLP-1 weight-management support. Most of them have been sitting on the question for months. Make it clear and relatable without giving medical advice. YOUR APPROACH • Use your normal voice and content style. This should feel like your story, not a scripted advertisement. • Open with a real question, concern, or misconception you had before you started. • Share what led you to Bliv, what the intake or care experience was like, and what someone considering it should know. • Show the experience visually where it helps, but never display medical records, account details, prescription labels with personal information, or anyone else's private information. • Keep it focused on your genuine experience. We may suggest factual or compliance edits; your delivery stays yours. WHAT YOU'RE PAID A flat fee per accepted video, set by your follower tier and stated in your offer before you start filming. It does not change based on views, clicks, or sales. See the Creator Compensation Schedule for the full tier list. BEFORE YOU PUBLISH • Submit the video to Bliv for written approval. Do not post until you have it. • Publish on the platform where you have the most followers. On Instagram, publish as a collaborative post with Bliv. • Use that platform's paid partnership or branded content label. On Instagram, also include #ad and #partnership in the caption. • Grant the partnership ad access we need to run the video as an ad from your handle. CLAIMS AND SAFETY GUARDRAILS • Do not promise or guarantee weight loss, health outcomes, timing, diagnosis, or a cure. • Do not state a specific amount of weight lost or a specific results timeframe unless Bliv has approved the exact wording in writing. • Do not call compounded medication generic, FDA-approved, FDA-reviewed, FDA-evaluated, or equivalent to an approved branded drug. • Do not imply that Bliv compounds the medication itself. • Do not make superiority claims about competitors, or imply Bliv replaces emergency care or an in-person medical evaluation. • If y
